PalAssist

PalAssist is a 24-hour accessible service that is free for anyone who has a life-limiting illness or condition, and/or their families and carers (subject to any costs that may be imposed by a client’s telephone service provider for 1800 number calls). PalAssist is operated by a team of nursing and allied health professionals. We understand that every person’s journey is different, and offer individualised support and advice to our clients.

Caresearch is a website which provides trustworthy information for patients, carers, families and communities about all aspects of care at the end of life.

Caring@home

caring@home has resources available to support people to be cared for and to die at home, if that is their choice. Resources are applicable Australia-wide for community service providers, health care professionals and carers to support carers to help manage breakthrough symptoms safely using subcutaneous medicines.

Palliative Care Queensland

Palliative Care Queensland is an independent not-for-profit peak body representing palliative care providers, consumers and their families and those with an interest in palliative care in Queensland. Their website contains useful information and resources for the community.

Carer Help

Carer Help is a website that provides information to people who are caring for somebody who is dying. Carer Help has a series of pathways (depending on where your loved one is on their end of life journey) which provide practical suggestions on where you can get help as a carer and things you need to maintain your own health whilst in a caring role.

Gather my crew is an online help roster. Friends and family want to help and this can be a practical way for you to organise their efforts.

The Australian Centre for Grief and Bereavement (ACGB)

ACGB is an independent, not for profit organisation and is the largest provider of grief and bereavement education in Australia.

Palliative Care Australia

Palliative Care Australia (PCA) is the national peak body for palliative care. The PCA website includes brochures and fact sheets about palliative care that you can download.

Advance Care Planning

Advance Care Planning is very important for all people who are diagnosed with a life limiting illness, or are frail and elderly. Advance Care Planning involves thinking about and making choices about your future health care, whilst you are able to do so.
Planning in advance helps those who are close to you make health care decisions on your behalf, when you are no longer able to make those decisions for yourself. These decisions can include, where you would like to receive care, what your treatment wishes are and who you would like to provide care.
There are several Advance Care Planning documents in Queensland:

Advance Health Directive

The Advance Health Directive is a document that states your wishes or directions regarding future health care for various medical conditions. It only comes in to effect if you are unable to make your own decisions. The Advance Health Directive requires involvement of your doctor and a witness (who must be 21 years or older and be a justice of the peace, a commissioner for declarations, a lawyer or a notary public).

Enduring Power of Attorney

An Enduring Power of Attorney is appointed to give legal power to make decisions on someone else’s behalf when they no longer have capacity to make decisions. Enduring Power of Attorney can be given for personal matters (including health care), financial matters or both.

Statement of Choices

The Statement of Choices is a values based advance care planning form used throughout Queensland. The form focuses on a person’s wishes and choices for health care into the future.

Acute Resuscitation Plan

The Acute Resuscitation Plan (ARP) is a Queensland document that can be used in any health setting in Queensland. It is a medical order initiated and completed by a medical officer (through the completion of the ARP form). The ARP form is completed in collaboration with the patient or the substitute decision-maker (if the patient has lost capacity).

 

West Moreton Health has a dedicated team of Advance Care Planning nurses who are able to talk to you about advance care planning and facilitate you completing your forms. You can contact the Advance Care Planning via the Ipswich Hospital switchboard (3811 1111).
